Player Evaluation Initiation

1. Download the player evaluation form by clicking the "Field Player Evaluation" or "Goal Keeper Evaluation" button below.
2. Have the player complete the tab labelled "Player" independently of the parent(s).
3. Have the parent(s) complete the tab labelled "Parent" independently of the player.
4. Email the completed evaluation to lislefcdoc@outlook.com.
5. Your coach will follow up with you when they have completed their portion to schedule a sit down to go over the form.
